Joints can become worn with time or due to trauma or injury causing pain. This is generally known as osteoarthritis. When this is severe enough to cause debility in the hip joint, the whole joint can be replaced with metal components. A hip replacement may also be needed after a fracture of the neck of femur but this is usually done as an emergency procedure. In either case, the operation is done through an incision in various parts of the hip or buttock area.
The hip joint is an example of a ball & socket joint where the ball is the head of femur (or thigh bone) and the socket is the acetabulum (or pelvic bone). Therefore, a prothetic or artifical hip joint consist of a "cup" that replaces the hip socket, a "ball" that replaces the head of femur and a "stem" which is the long portion which is cemented into the femur to hold the ball in place. These components are usually made from high-tech metal alloys or ceramics.
The operation is usually performed under a general anaesthetic, which means that you will be asleep during the procedure and therefore will feel no pain. Sometimes, the hip replacement surgery will be carried out under an epidural (spinal anaesthetic using local anaesthetic that is injected into the lower back) that completely blocks feeling from the waist downwards but means that you will be awake during the procedure. You should discuss the options with your surgeon since some patients find being awake during an operation traumatic.
Once your anaesthetic has taken effect, your surgeon will make an incision, usually around 20-30cm (8-12 inches) long along your hip and thigh. The length of the incision may be shorter depending on the technique your surgeon is using. For example minimally invasive surgery requires much smaller incisions.
The top end of the thigh bone will then be removed and a ball on a stem will be placed into the thigh bone - this is the replacement hip
The hip socket will be hollowed out to make a shallow cup and an artificial socket will be placed into it. The artificial part of the thighbone can then fit into this socket.
Your surgeon will then close the incision with stitches or clips and cover it with a dressing. This is the end of the operation, you will usually be taken to a recovery room before returning to your room or ward.

How You Will Feel:
Total hip replacements are mainly done under general anaesthesia but can be done with a spinal or epidural anaesthesia.
Strong pain killers will be required for a few days after surgery, followed by an extensive period of rehabilitation with input from physiotherapists, occupational therapists and others. However, the outlook after rehabilitation is usually very good.
